Weird episode. I think I liked it, but that may only be because Desmond is one of my three favorite characters. I wonder how the flash sideways hating folks are going to respond to what was almost entirely a flash sideways episode?
Question time: how exactly are Desmond's flashes working this time? Does he pass out in the other world while he's active in the other, kind of like in The Constant? Did he just pass out on Sayid when he flashed back to the Sideways world for coffee with Penny? Speaking of coffee, are Juliet and Sawyer going to finally go dutch at the same coffee shop? Is that how Sawyer senses the love (which is apparently the Harry Potterish key to Lost) and realizes the Sideways World is a sham? Finally, what the **** is up with Eloise Hawking? Why is she the only person besides Desmond who seems to know what's happening with flashes between time and now worlds?KH FOR OWNER!

That episode was ****ing awesome.
I didn't quite understand all of it, I have some of the same questions you do, although I suspect some of them aren't particularly important. But it was terrific, very affecting. Right up there with the Locke episode as the best of the season so far.
Alternative Desmond is going to unplug everyone from the Alternative Matrix--he is Morpheus!
The idea that the Alternative World is what happens if Smokey escapes is becoming more and more solid; that is is some sort of Matrix where he gives everyone what they most want (Des wanting Widmore's approval). Not sure what will happen to the Island people when their Alternatives are confronted by Morpheus Desmond, or how it relates to him being special regarding EM, but ****, that episode ****ing ruled."My nation is the world, and my religion is to do good." --Thomas Paine

But it was terrific, very affecting.
For some reason, I was affected much at all. That's probably due to the familiar reason that I don't think the Sideways World is going to survive (especially after the end of tonight's episode), so whatever happens in that world that doesn't have some bearing on the Island World seems pointless to me. Yeah, Desmond met Penny again, but so what? We already had numerous better Desmond/Penny moments and this one won't even exist when the Sideways Worlds ends.KH FOR OWNER!

Dominic Monaghan rocked the **** tonight. You couldn't take your eyes off him. His description of seeing love, while dying, was heartbreaking--an explanation of, "I was supposed to die." His conversations with Des about having a choice, especially, were terrific. The image of him in the car, drowning, hand on the window... wow. You knew they were going there, but it was still heartbreaking.

Remember, Eloise said to Des, "You have what you most want in the world, the approval of Charles." (Or something like that.) And, that was true--of Island Des.
And yet, getting what he wanted did not make him happy; getting what he needed--Penny--may do the trick instead. Why should that matter to the audience? The only way the audience cares about Sideways Desmond is if the producers destroy a world in which Desmond already has Penny (and all the awesome moments involved that we watched over five seasons) in favor of a world where Desmond has a chance to go after Penny...KH FOR OWNER!

I'm not saying I like it, but I think the thought process would go something like this:
These people have been trying to get off the Island/go back to their old lives/etc., but they did not pay attention to what they truly needed. This is a way to show that their old lives were empty, that they truly did crash for a reason.
Don't like it, but that's a possibility. I certainly think it more likely that the Sideways World will go poof once Des unplugs everyone from Smokey's Matrix, and that the (Sideways) candidates' realization that their Sideways lives, while on the surface more appealing than their Island lives, are a sham. Perhaps the Island candidates must come to the same conclusion as well, and make the same choice.
EDIT: Also, the Island World is a world where Des has Penny only after blowing it for a decade because of his obsession with not being good enough for her father's approval, and even after regaining her, he hides from Widmore lest he takes Desmond's life away."My nation is the world, and my religion is to do good." --Thomas Paine
