1) Dutch, so they aren't really looking at obese people the way we do it here.
2) They're only counting health care costs. People 'cost' society by NOT producing positive value (through work, for example); so if you're obese and die or are forced to retire at 50, for example, you cost society 15 productive years. You're looking at potentially billions in reduced productivity.
3) It uses a simulation model, which means it's only as accurate as the input assumptions... which could well be faulty, either accidentally or by design (ie, keep adjusting the parameters until you get the results you are expecting).
