Nope. Pres. Polk did a deal with them. They formed the Mormon Battalion to protect California from the Mexicans during the U.S. war with Mexico, and in exhcnage Polk let them settle in Utah.
Trivia: The Mormon Battalion is the only unit of the U.S. military ever to have a religious designation.

I honestly don't know the fourth. Hawaii was an independent absolute Monarchy before the US took it over, Texas was independent for like 13 years, California for only 2 years, but the fourth?

Texas was only really independant because the United States didn't really want it thinking Mexico would get angry if we annexed Texas (then times changed and we wanted to get Mexico into a war.)

More precisely: Hawaii was a monarchy before Yankees overthrew the Queen and set up their own republic. Later, the Hawaiian Republic was annexed by the U.S.
As for Vermont, Wikipedia describes how it was a disputed territory between New Hampshire and New York. After the Revolutionary War, it declared its independence but later, joined the United States as our fourteenth state
