Meaningless though, all systems have limits to what healthcare they can deliver and overall level of funding so all are therefore somehow 'rationed'.

Comes back to a fundamental principle. Should people who can afford better coverage be able to purchase it? Many businesses work that way - 20 percent of the customers fund things for the other 80 percent. Health care is no different. People who are willing to pay top dollar keep the profit margins up so that the hospitals can afford to spend on less profitable care.

Forcing all private insurers to have the exact same coverage is going to put them out of business. A business has to be able to establish a competitive advantage. When the government sets the price and the coverage, what's left? There's simply no point to it.
