Yeah, I had that problem. It's also a pain to have to change all of your "teh leet" equipment just to survive in the Thirst, but I suppose it's realistic enough so shouldn't complain.

Quite often I WANT to kill those who surrender in the OC, and smashing them before they do so saves me from evil points I would get otherwise.
Seriously, look in the prison of the peninsula district. I killed evey ****ing prisoner up to this point. Why would I leave that half orc Kurdan Fenkt, who's the worst of all, live?
Or Meldanen, the sorceror in blacklake. I had the nerve to break into his home, steal all his belongings, kill all his guards and even a couple of Apprentices, and in the end I let him live and make him pull out his tooth? WTF? No chance.
Zor the Minotaur. Same thing as with Meldanen. Why would I let him live just because he tells me a whiney story? He might lie and most likely does. The best thing would be to take hime alive and bring him before the court. But since the game does not offer this opportunity, goodbye Zor.

I'm pretty sure I killed Kurdan Fenkt and Meldanen through the options in the conversation. I do enough good things to offset any evil points I get. The problem I worry about is if not talking to the npc leaves a quest open or not getting an item.
Also, killing the werewolf boys needlessly is something I'd rather not do.

I was really angry when I couldn't finish a couple of quests because of bugs.... I think it was in the second act of the original campaign... One in which you have to kill a wolf on a cave... I convinced it to leave, and no matter how high I got my cha I couldn't persuade the farmer to believe me. So I went back to kill the wolf and it wasn't there... gah.
And I was supposed to give back a troll/ogre/whatever chieftain's head to some guy in the archeologist camp's barracks (i think) and th edamn guy was busy attacking an (invincible) dummy, and kept telling me he was too busy when I tries to give it back...

I'm not still playing it. I am playing it again. As for why, shrug, I just had cravings to play NWN and most player created modules are yet more buggy and demand installations incompatible to each other, so the OC is it. And if you ask why I don't play SoU/HotU, well, I am playing it too, I have a wizard there in HotU chapter 3.
I don't put much effort in it, though, I play it when I don't have (or want) anything else to do. Every week perhaps 1-2 zones/dungeon levels. I'm in Mutamin's challenge now for several days already.

I'm soloing it on hardcore D&D. I probably could turn on hard, because combat's getting too easy. But then, I am a sorceror. Btw I never took a henchman in this game, ever. In low levels I relied on my familiar (pixie) for some extra damage and sometimes summons as weak meatshields. Now I am level 12 and didn't need that for ages.
