The way US sanctions typically get you is though banking. The US can demand that any bank doing business in the US must abide by the sanctions. Thus, even though the sanctions are on country X, anyone in country Y who wants to do business with country X must do so without using any banks that do business in the US. In practice, this makes US sanctions pretty hard to bypass. I figure the EU is counting on something similar.
