I haven't talked much about Manning in this thread because I don't really have a strong opinion on her crimes or predicament or what have you. I do have a strong opinion about gender stuff, though. Sex reassignment surgery and hormone therapy are treatment options for gender dysphoria. They're not recommended in all cases, but they're available if a doctor deems them prudent.

As far as I'm concerned, this is no different than, say, a corpus callosotomy. There are many ways to treat seizures, but sometimes seizures can be stubbornly treatment-resistant. As a last resort, doctors can sever the two halves of the brain, and this can provide considerable relief for the patient.

Similarly, gender dysphoria is ultimately a neurological condition that sometimes doesn't respond to more mundane treatment options. In those cases, somewhat drastic measures are called for. Sex reassignment and/or hormone therapy can provide considerable relief.

Btw, it is gender dysphoria that is a psychiatric condition, because it is gender dysphoria that contributes to the psychological pain doctors see. Self-identifying as a gender other than your biological sex is not the psychiatric condition. That's just behaving in a way that doesn't accord with our cultural standards concerning gender.
