The Republican politicians in the red states are basically bullies in their behavior. Not only are they refusing to expand their state's Medicaid program, but they're also making it as difficult and convoluted as possible for groups and organizations to help people with choosing and signing up for coverage on the federal government's Affordable Care Act market website.
And before we get into the Canadian model - do remember that each province controls their own healthcare system, and coverages vary across provinces. Even the Canadians aren't stupid enough to federalize control of health care. I see no reason why health care in TX has to be the same as it in other states.

It's an additional cost of operation for every single health care provider in the state. Texas has said no - because the funds would result in a increase in medical costs in the state. Texas has also blocked implementation of Obamacare for the same reason.
