I've always understood it as lying, plain and simple. But I have wondered. The commandment you should not kill excludes killing in war(we know because of the wording), so for all I know there might be exclusions in this commandment too. I mean, if I could save someone from the nazis in WW2 by lying, I'd say that was ok. But I don't have anything biblical to back that up.
