There almost has to be. I've seen the pattern many times.

3) How does it work?

Damned if I know. I've never used my ISP address here, ever. It might be that context words (a la the google adverts) become associated with my name (SpencerH) and they get linked back to a cookie with my ISP address somewhere.

Obviously, poly is not to blame, but I have wondered if poly was a target for such searches maybe via the advertising pop-ups.

I do. I have made a dedicated trash-addy for all those sites I joined on a whim. I still have it, and I'll keep it as long as Caramail exists
