The primary method of determining a hostile individual is if the pull a gun when they see you.
Now, aside from that there are a few other ways. First, certain things are always hostile: raiders, feral ghouls, super mutants, vicious dogs, yao guai, mole rats, radscorpions, all wandering robots, deathclaws and such. If you see those, you are going to have to fight them or avoid them. You can use the VATS system to identify anything you see from a distance; you don't have to engage with it.
When you get closer, mobs have a bar that shows up in your compass. It's a green bar if they're (currently) well intentioned, red if they're hostile. Your "crosshairs" turn red when over a foe as well, if it's within effective range of your weapon.
