The Crusade of 1197 is instructive as it was lead by Henry IV of the Holy Roman Empire against the Muslims but traveled through Constantinople. There "the Emperor of the Germans was greeted and was welcomed with much honor by the Emperor of the Greeks".
The Bzyantines did declare themselves the Eastern Romans mostly to maintain long gone claims on western Roman lands but were effectively a Greek Empire run by Greeks and dominated by Greeks. There were other ethnic peoples in the empire but everyone knew who was in charge.

Well, maybe I got the plague wrong but, yes, there were numerous different outbreaks and, yes, some of them contributed. I will have to look up exact dates. There were dozens.Try http://wordforge.net/index.php for discussion and debate.
