People would pay a certain amount of medical expense out of their own pocket (or their HSA, most likely) and would then be covered for any expenses above that amount (which would scale based on income). It would be universal and cheaper than the current system thanks to proper incentives.

Alright, but it won't be cheaper for chronic conditions, drug prescriptions, and routine doctor visits because there will be a higher deductible. I guess you can scale it to payer bankruptcy. However, catastrophic health insurance plans don't generally cover these conditions at all. They were designed solely for young healthy people who lack other means of health insurance.
