#2 and 3 seem to be the culprits. I don't know how you switch off your brain -- my wife's an insomniac, and has the same trouble with worrying thought -- but it's the thoughts, not the sleep, that's the problem
As for #3, it's best to do something very low-key and passive in the hour or so before sleep. Reading in bed works well.
