You know, thinking back to my undergrad years, I seem to remember a number of Byzantine rulers (emperors, regents, generals, hangers-on) who were stark raving mad. Gouging out the eyes of their siblings or castrating them or, if they were in a bad mood, both... unfortunately, I can't recall any names. It's been over three years since I had that class, and I sold all my books.

The Somoza familiy dictators in Nicaragua would be my favourite pick. Anastasi Somoza, who was in office form 1937 to 1953, if memory serves right was the one who made FDR said: "He may be a son of a *****, but he's our son of a *****".

The Somoza family would be pretty good, just in terms of far out corruption, especially with such things as the House of Dracula. But I'm not sure I'd term them psychopathic.

How about "Daddy" Duvallier, of Haiti? He was a voudun priest in addition to being one of the nastiest dictators of Haiti (which is saying something.

The problem with that was, one, Lenin didn't have dementia, he had a stroke. If you read his writing from that period you can tell he's still got a sound mind. Two, he was no longer in power, as he was convelescing. It was in this year that Stalin began his take over of the Soviet Union, which would be completed six years later.
