In my experience playing huge maps with continents, the above is true: that continents have monopolies of x number of resources, but within the continent itself a single luxury may appear at either tip, meaning in order to have a "true monopoly" on a luxury you must often conquer the entire continent.
If gems appear on continent A, moreover, they will NOT appear on continent B. However, they may appear numerous times in almost any location on continent A.
