This case (the murder in Texas) proves once again what I've been saying for a long time...you can't buy good psychiatric care in this country. The vast majority of cases of depression are managed by family practioners and internists such as myself. Though the medical societies and insurance companies who approve of this deny it, these generalists usually don't have the time to devote to psychiatric care and most don't have the skills to manage more serious cases either. Then again most psychiatrists trained in the US don't either. The problem is that we've become overly reliant on medication to treat the patient and we ignore the importance of communication. It's true that with respect to most types of mental illness "the talking cure" is pretty useless. Putting someone on the couch and having them free associate or recount their childhood may even be counterproductive, but you have to talk to someone to determine if there illness is well controlled or if they're suicidal. Mentally ill people often have a great deal of denial, so the process of getting to know them well enough to assesss them is very time consuming. A GP's office is not a good place to do this, there are too many distractions.
There is a relative deficiency in numbers of psychiatrists, but I'm not sure that it matters. Most of the psychiatirists that I've worked with recently seem to be more interested in administrative duties and not very sharp with regards to assessment skills. I've sent overtly suicidal patients to see some of these guys in the ER only to have them bounce right out with a handful of pills. Sorry, but in my handbook suicidal ideation is an automatic hospital admission.

OTOH do we know that this woman was currently depressed at the time she killed the children? I didn't interview her, so I don't know whether she was currently depressed or just pissed off at her kids.

Too bad I had to miss Dookie running to mommy. You remind me of a kid I knew in school once. He was always getting the **** kicked out of him for one reason or another. He never figured out that his smart mouth got him into trouble. He was always running and telling and would get it even worse after that. Until one day people just started ingoring him. People are still not paying attention to him 7 years later. If you can't take a little abuse then stop being a jerk.
