My vote goes for stacked combat a la CTP2. Of course a few new civs along with a handful of new units & technologies (maybe even a new religion) are par for the course. What do you most want to see in the cIV expansion pack?

I hope for some new features and tweaks to existing ones, plus some extra civs & units. I doubt fully-stacked combat will be in, given the scope of that change.
Great Generals would be nice.
Tweaking the UN would be good, too (I'd like there to be a way to defy the UN - even the final vote, like in MOO2).
I'm sure I'll think of more, but those two things spring to mind for now...

Since it's already possible for cities to flip to another civ, include a probability of them splitting to form their own civ especially if they are on another continent from the original civ's capital. Simple and easy implementation of civil war.
Some more unconventional warfare tactics. Spies plant disease, plant nukes, or steal tech. Maybe an ancient unit like a diplomat which can do some spy activities like destroying production in the early game.
Tactical nukes able to be put in subs. Have same power as current ICBM but immune to SDI.
Upgrade ICBMs to something approaching the power of the planet buster from SMAC.
Cruise missiles which have an insane collateral damage/city defence bombardment value but are automatically used up in the attack.
