And technically it's not MAD at that point, just MD. MAD is a position you can only be in when all sides have enough nukes to destroy each other but haven't done it yet ...

Yes, but Mutually Assured Destruction requires each of us to have enough nukes to kill *all* opponents. In this game, it's impossible to have MAD with anyone having 5 cities
Really, MAD only applies in a 2 or 3 player scenario. 4+ players need to each have a 'stockpile', which only lasts one turn, AND to have no bunkers. That's another issue that prevents MAD in this game.
