The most common case (at least for the porn I watch) is a chick with breasts and a comically large set of **** and balls, such that it is impossible to tell if she's got a vagina.

A rarer borderline case is the woman who can enlarge her clitoris to the size of an erect penis - she's generally considered to be a woman with a bizarre clitoris instead of a dickgirl.

I watched about half of the movie, quit at the halfway mark because it was both creepy and repetitive.

You need to see the ending. Anwar Congo takes them back to the place where he committed most of his murders in an attempt to explain what happened. But he just can't do it- he starts retching uncontrollably and coming apart at the seams. The camera just stays fixed on him without cutting, and lets him tear himself apart. It's an incredibly powerful sequence, and shows that he just can't live with what he did.
