When did you become an intern?

My heart pumps purple panther piss for you. The last 4 hours will be fairly wasted, and even the USAF never had us on prolonged 12 hour shifts. Efficiency goes down and safety concerns rise, which makes the medical profession fairly alone in their ignorant approach.

I think the point with doctors is that handing a patient over increases risks more than having someone do shifts.

When I skip nights, getting to 6 is easy. Then the next couple of hours sleep really hits, with 9 being the worst. If I didn't cave in by 10, I'm OK and pretty active / psyched until 5-6 PM, when the lack of sleep starts hitting me again, but the rest of the evening is fine. Keeping up past 12 PM is really tough though, but I haven't really tried to go for so long these past couple of years. Trying to do any sort of physical activity after a sleepless night is impossible.

Actually back in the day for an intern the overnight shifts were more like 32 hours. You have to realise that originally, back in the late 19th - early 20th centuries, the term "intern" meant exactly what it sounded like. Interns were literally sequestered in the hospital they worked at and were let out only for a few hours per week - much like nurses were at the time. The way this happened is that a bunch of attending physicians at Johns Hopkins University observed that it s***ked to be called into the hospital at any hour of the night to admit new patients or tend to one who had taken a turn for the worst. They also complained that the young whippersnapper newly graduating doctors (taught by them) "didn't know jack-s**t" about practicing medicine in the real world. They combined the need for extra practical education for newly graduating physicians and their need for a little "me time" and came up with the internship - because slavery had been recently made illegal.

This is, unfortunately, not unique to the medical profession.

My general rule of thumb is that after 11 hours you start making mistakes you normally wouldn't make. And most effort after that is just a waste of time since the work will just have to be redone anyway. I've always thought that the long shifts in the medical profession are wrong and should be considered criminal.
