          Only if you're so poor you make it on to the exchange. Other wise you have to go buy it at market rate and, yes, they'll charge higher rates for pre-existing conditions.

              You must have a reading disability because he specifically said that is a problem in NY state pre-ACA. Something which the ACA fixes.

                    The phrase "no more exclusion based on pre-existing conditions" is a bit vague though. Is it "exclusion" to charge someone extra? They're not being denied coverage if they have to pay somewhat more.

                      Community rating is different from guaranteed issue (ie. banning exclusion based on pre-existing conditions). The former prevents insurers from charging higher premiums for a person based on pre-existing conditions (and many or all demographic characteristics), while the latter prevents insurers from refusing to cover patients with expensive pre-existing conditions altogether.

                                Largely irrelevant, as your article goes on to explain, as UHG has a very small presence in CA's individual market. Kaiser, Anthem, and Blue Shield have almost 90% of the market share of individual policies. Health insurance is a state-by-state industry where there are huge differences between states - it's not like cars where you can say that GM and Ford are the big companies or whatever; UHG has a huge marketshare in some states and tiny or none in others.
