From my experience with my father, who's had two heart attacks and a quadruple bypass, the most important thing to remember is to ****ing stick with the changes you need to make in your lifestyle. You have to change your diet, you have to be fit, you have to take your medicine, you have to see your cardiologist regularly, and you have to absolutely not fall back into old routines.
