What do you do during daytime, Tass?

If you aren't doing anything (it looks like it, given your sleep pattern), then do a physical activity. The best thing is to do a mnual job, which will keep your body busy all day long: you won't have trouble finding sleep afterwards

I find sudoku puzzles excellent for relaxing my mind. Do more challenging ones and start about 15 mins to half an hour before you want to sleep. I do them in bed as a relaxer and it works excellent

Also it seems like you are often getting 6-7 hours of sleep, its just at the "wrong" times. Simply you should no longer allow yourself to sleep in. If you want to sllep at midnight and get up at 8, get up at 8 regardless of when you get to bed. Be active and stay active all day . I would be shocked if it doesn't work. If you do manage to go to sleep on time, don't allow yourself an oversleep the next morning.

I find sleep to be heavily routinized for me. I go to bed between 10-12 at night and am up before 6 even if I don't set an alarm. I fall asleep each night in seconds

          You have to force yourself?!

And as for an answer to the original post: get a job, that constrains what hours you can sleep...

That is a pretty good sleep pattern. Mine is like this:

go to bed at 3 pm, wake up at 6 pm

go to bed at 6 am, wake up at noon

go to bed at 5 am, wake up at 8 am

go to bed at 9 pm, wake up at 4 am

            etc
