I am having a large problem with sleep.
I am only able to get to sleep at a godly hour using melatonin. If I do not use melatonin, I will tend to sleep far far later in the day, and adjusting my schedule through various methods has not proven effective (at least, effective enough within an acceptable timeperiod).
1) What's your caffine intake like, and has it changed significantly lately?
A cup or two of tea every day.
2) When you wake up in teh middle of teh night and can't fall back to sleep, do you find you're thinking about anything in particular?
I think about how little sleep I'm getting and stress that I won't be able to function the next day.
3) What do you do for the hour or so before going to bed?
Read, rest, etc
4) How much outdoor activity and/or exercise do you get in a day?
I try to exercise once every day for about an hour, but I don't always get the opporitunity. Outside of this, very little (other than what one would usually encounter during the day)
I was told to lay off the booze (beer) cause that is what they say wakes me up out of a dead sleep. Course i was an insomniac before i was an acoholic and i dont see a difference in waking up patterns

Being dyslexic is hard work. I don't even try anymore.
It sounds just like the problems my mom had. She always slept to little, woke up in the night etc.. Just like you did. She got some pills, not sleeping pills but pills that helped her relax before she went to bed and now it is much better. She had exactly the same symptons as you had. Can´t remember the name of the medicine though :\
