I have no idea what you've tried, but in general if you are having a software issue I'd try the following.
1. If you are running Windows XP go to Start then Run. Type msconfig then hit enter. On the general tab under selective startup uncheck a box labeled 'Load Startup Items'. Then click ok. If you are running W2K let me know and I'll find the link for msconfig for w2k off of Microsoft’s site. If you are running Windows 98 or Windows ME then that’s your problem and you can stop there.
2. Update to the latest video card driver direct from the manufacturers website. This should be either www.nvidia.com or www.ati.com (I'm sure you are well versed on this but if you need details let me know). If you are running a Dell, HP, or some brand name computer don't get the driver from them as its usually outdated. If your video card isn't an Nvidia or ATI brand then that may be part of the problem.
3. Run Adaware and Spybot to eliminate spy ware programs that may be running.

Download and run them both.
4. Virus Scan. If you have antivirus software make sure it’s up to date and run a scan. Start it up manually as it should have been disabled from starting up automatically in step 1. If you don't have an AV program (and I don't, just stay away from pr0n and keep the firewall on) then use this free online scan here:

I've used this previously when I accidentally left an open share on once. It cleaned up the virus I had got because of it.
5. Run the game. Set all graphics to 'low'. Use low resolution textures. Disable the sound (for now). You may have to exit and restart the program. Start a new game. Play on a 'small' or 'standard' size map for now. Stay away from larger maps until you are stable.
6. If the game still crashes on a small map after all of that then run a scandisk and defrag of the drive the game is installed on. If you get to this point let me know.
All of these things should have been suggested to you by now I would hope. But in case they were not. Step 1 and 2 are the big ones that can help. If you have to do step 6 then you are getting to the point where it may be a hardware issue.

Happily, I found patch 1.09 on Poly's archives (thanks Dan and Markos) so I can play the game with that patch and your memory mod.
