Instead of doing a unique promotion for only certain types of terrain (as seen above with guerrila & woodsman for instance), why not do a generic "Terrain Defense Bonus" promotion that allows the player to choose the terrain he wants a bonus in? This way, and island-based civ could get defense bonuses in coastal tiles with ships, a desert-based civ could get defense bonuses in desert tiles, etc. To expand that further, add a "Terrain Attack Bonus" that gives bonuses to strength when attacking in that terrain, and a "Terrain Movement Bonus" promotion would grant a bonus to move in certain terrain. It's a bit more generic and looses some flavor this way, but has much more flexability.
Also, I suspect that the foot image with a red "+" sign by it is a bonus to healing rate outside your borders.
