I've never had any of the "You have traded with our worst enemy" or "You made an arrogant demand" go down, they always stayed the same, even though I never traded with them again
Also the negatives should go away if something happens between those two civs. Here's an example:
China and Mali are friends, I decide to attack Mali, then China starts to hate me. Then at some point later Mali attacks China, but I still get the "You attacked our friend" diplomacy hit from China, even though they hate each other
This space is empty... or is it?

Numbers are numbers, there's not going to be any variation in how they're calculated. Mathematics doesn't allow for multiple answers.

Yes, but you need to consider it from their point of view. It's not a matter of whether you attacked someone they liked or didn't like, it's a matter of how 'trustworthy' they consider you. If you have a habit (and even once can be a habit) of attacking people they like, that's going to annoy them, regardless of whether they grow to dislike the other civ.

Admittedly, however, those penalties shouldn't last 'forever'.

They shouldn't last forever, but you bring up the point of "reputation." I'd forgotten about it, but that does make sense.

I just wish we had the thing back where your foriegn minister tells you "Our name is mud." or "We have a spotless reputation."
              I don't know what I've been told!

There's no need to now. If another civ is showing nothing but green when you contact them, then your reputation with them is spotless. With relations being calculated on a civ by civ basis, that kind of information is meaningless.
