I'm sorry and I'm not saying this to vex you but the US is not a model I consider as a paradigm in health care services.
So you could be surprised but it doesn't matter much.
Norway OTOH is. It's having a universal health care system that covers the whole population through taxes. And it seems to be working ok.

A judicial inquiry is taking place at a state hospital in Athens where a 67-year-old heart patient died after allegedly being denied timely surgery by a doctor seeking a bribe.
That's the insentive I got for posting this poll btw.
This is attrocious, a deliberate murder. That "doctor" should go to jail for the rest of his life.
However there seems to be an extrainstitutional equilibrium reached in Greece that now with the crisis it's breaking down.
I remember in 2006 a global survey put the greek medical system above that of the UK. I'd say that was a good achievement.
I dread to look where it is placed now.
Bribes are not "forced"
Everyone is entitled to free health care and he gets it.
BUT
(here's the big but)
there is very often a negotiation so the doc could get something more.
Bribes are effectively annuling free health care here and coupled with the crisis that is hitting health care and other things, things are nightmarish, if this is an example of what is going on.
