Prior to widespread access to roads, canals and (eventually) rail travel, the vast majority of people lived and died within x miles of their birthplace (where x was a very small number, say 10), never venturing beyond and knowing only those things about the outside world brought to villages by traveling minstrels and official representatives of the King/Duke/Archbishop.
I can only imagine the skewed viewpoint those ignorant peasants had of the larger world. It makes me sad.
