Some health insurers pay for your basic gym membership and/or give you a discount on your premium for going to a gym. Only caveat is that you have to go to the gym for a set work out a set number of times each month.

I plan to take up smoking and living like an unhealthy slob when it comes to the assessment of my pension annuity. Then once I have that big chunky annuity I will switch back to my healthier lifestyle.
So, erm, when are you going to switch to the healthier lifestyle in the first place?

Yep. The purpose of any healthcare program should be to provide healthcare to the sick. In the U.S., our program is designed to maximize profits for the insurance companies.

It has nothing to do with individual responsibility.
On the other hand, someone who doesn't take care of themselves will get a lot more medical costs than others (on average).
Finally, though, many medical issues are no ones fault. They are caused by bad genetics, birth, accident, or something otherwise not controlled.
Smoking, not exercising, stuff like that is controlled.
