    Audis used to have a problem with unintend acceleration due to the spring on the throttle being to weak. Most cars now days have an electric throttle instead of the old mechanical throttle though so I doubt this is the problem in this case.

      and they used to make a stupid brake design with the discs not at the wheels but closer to the middle of the car. pain in the ass audis

        There's another possibility, ask her if she has cruise control and if so, did she have it on just prior to braking. Its supposed to shut off when the brakes are hit and you'd know if it didn't as the engine kept trying to go 60 mph or whatever while the driver is trying to stop. But it might have shut down when the brake was applied and then resumed control when she let the brake off. That would cause the car to surge forward very fast
