Terrain Defense

Hi, I've been looking in the book & www.civ3.com but can't find the answer to my question.

All I want to know is what Terrain Defense does. Let's say that I have a 1-2-1 Spearman on Grassland (10 Defense) and another one on Hill (50 Defense). What does the number actually means.. is it 10% more defense on Grassland & 50% more defense on Hill? That would basically mean that my Spearman on Hill is now 1-3-1 ? or am I wrong? Let's say that the Spearman is Fortified.. does that increase his defense too??
