I may have to re-install CIV III when I get Conquests because of a recent OS change. If I do so, will I have patch CIV III and PTW or will Conquest update that automatically?

If you want to play Civ III PTW (and not Conquests) you should patch with the latest PTW patch. You will be able to run PTW only (not Conquests) by finding the PTW executable.

If you are playing Civ III Conquests, the shipped product will be the latest build. However, its very likely there will be a Conquests patch within this calendar year. Programming problems are always more fully uncovered when the product is in full release.

Actually Conquests will include the latest CivIII and PTW patches. So you don't need to install any of the previous patches after installing it, even to play PTW or vanilla Civ.
