Lazy Workers

I'm about midway into the game. My workers should be building railroads and terraforming the land around my new cities. Instead, they are just fortified in my cities. Why is this happening? This happened in another of my games, so I started a new one to see if it would happen again. Both of this games are scenarios that came with the Conquests Expansion and I did not mess with them. I also got the new patch. Does anybody know why this is happenning?

Well it would be helpful to name the scenario and maybe post a save.

In practice the main reason workers go to the cities in mid game is they are on automate and have no work to do at the time.
They will head for cover if threats are near, but should not stay.

Can you move them out of the city and have them do task or park in a tile? Is plague in the scenario?
