I lost count how many attempts I have made in generating an ideal map using Smartmap. So far, every random map I generated using Customized and Smartmap have produced ZERO floodplains.
Is this a bug that others experience with Smartmap?

I don't have any experience with Smartmap, sorry, but I am suspicious of the map generation in general in cIV. Balanced always ends up as Pangaea for me. What if you want continents AND islands?
Will9, once upon a time, in the not-so-far distant past, I remember when I was able to generate maps with floodplains using Smartmap.
But at some point, quite some time ago, when I uninstalled Civ 4, then reinstalled it with the official patch, and reinstalled the latest Smartmap version, I no longer get any floodplains; I only get desert along all the rivers now.
I'm disappointed with this. I'm sure others are having the same problem?

I still use smart map and I get flood plains. Though I feel they are less common than they were in the "not-so-far" distant past, they are still around.
Infact, I remember I used to get flood planes on grasslands and plains and stuff.
For continents & islands, Terra is superb!
Of course, the New World has no civs on it. It'd be great if there was an option for that.
There was a map script posted at CivFanatics recently that generates a terra map but places civs on both continents. I haven't used it yet but the idea behind it is good.
I've found that fractal generates a nice mix of continents and islands.

Yes. SmartMap Strict uses smartmap feature generation but won't place anything in places that are unusual under standard terrain generation (such as flood plains on grassland). You need SmartMap to get floodplains on non-desert tiles.
Has the problem extended to no flood plains being generated anywhere, was that always the problem, or is it that no flood plains are generated on non-desert tiles?
