You can only pick your state religion to be Islam if you have Islam in one of your cities
I've been in a network game where two other human players converted to a religion for which they did not have a city of that religion. We were in the same team though, which may effect that distinction.

My understanding of this is that religions can spread not only from their holy cities, but from any city in which they are currently present. Hence, it is possible for a religion to spread via a third party, hence there is no requirement for an open border arrangement with the founder.
In other words: If you trade with someone who in turn trade with someone else, then that second "someone else" may end up with your religion even if you've never even heard of them.

see blake's post above. there's no need for ANY open border agreements. religion can spread through POSSIBLE trade routes (some poster indicated they may even spread without trade possible i.e. over oceans pre-astronomy, but i've never seen that)
that being said, i'm sometimes under the impression that IF international trade routes are in fact established, religion will usually spread along the very trade routes shown in the city screen

I was in a multiplayer game last night where a player with no religions set himself to Theocracy so I could not convert his cities to Buddhism. However, after I crushed his feeble stack of axemen with my macemen and he quit the game, the AI stuck with Theocracy despite having no religion in any of his towns until the end of the game. That seemed like a very poor choice for the AI to make, is it kept all of his cities free of religion, which is not a good thing in general.

No, I don't think it's possible for religion to spread by itself across an ocean without astronomy. Odds are, an AI was sneaking in missionaries with carvels and they just weren't noticed.
Basically, if you want to spread an early religion without using missionaries, the way to do it is to found costal cities, get sailing, and aslo build roads to your neighbor's cities and road networks.

That's incorrect. Religion spreads across *possible* trade routes, as far as I can tell from both my experience and the SDK.
Religion spread is checked (among other things) by a call to isConnectedTo() which checks getPlotGroup(), which I don't believe takes into account actual ability to use connections. I'm not entirely sure where plots get added to plotgroups though, so I could be wrong; that looked much more complex. Roading certainly affects it, but i'm not sure about other things (tech).

That dosn't match my experence, though. If that was true, religions should spread across the ocean quickly in the early game, but whenever you play continents, you always see religions A and B on one content and religons C, D, and E on the other one, until someone starts sending missionaries on caravels. When you play islands, you can find entire island civilizations with no religion at all.
I really do not think religion spreads along water trading lines before at least one of the two sides has the technology to trade across the water.
Religion spreads through all possible trade routes.
There are no possible trade routes across ocean before astronomy.
There are no possible trade routes between civs who are at war.
Therefore religion will not spread under those conditions.
