Failed to Initialize Renderer - Nvidia

I see lots of reports about Failed to Initialize Renderer issues with ATI, but I can't find this much with Nvidia.

I have the right version of DirectX, latest drivers etc. Has anyone solved this for Nvidia?

It gave me the error when I disabled Hardware Acceleration in DirectX then didnt when i re-enabled it. My problem is that the game suddenly started flashing my mouse as soon as it starts. Its driving me nuts and no one here can help
