After playing for 48+ hours... Civ IV is far different than the previous Civs in terms of scope -- fewer cities, more choices, smaller scale in terms of the "game board", even on huge.
Two different animals -- some people may play Civ III *and* Civ IV and keep them on their HD for completely different types of Civ games. Civ IV is a true sequel -- Civ II and Civ III were only slight modifications of the first Civ.
