Here's a mod idea I keep tossing around in my head. I call it "Modern Problems". The idea is to more fully represent the difficulties of a modern civilization. Right now, the modern era is rather similar to earlier eras. This mod would alter that.
New resources:

- Tobacco: -2 health in all cities with resource. +2 happiness. +2 to gold. Revealed by Economics
- Opium: -2 health in all cities connected by trade route (except yours). +25% to trade route income in all your cities. Revealed by Chemistry
- Cocaine: -3 health in all cities in all cities connected by trade routes (except yours). +25% to trade route income in all your cities. Revealed by Medicine
The drug trade is lucrative and destructive to your opponents, but causes diplomatic penalties. Opponents may close borders to you in order to stop the flow of drugs into their country.
Oil shortages:

- Oil resources that are worked have a random chance of disappearing on any given turn. (Similar to the random change of metals appearing in hills).
- Oil is less spead out and more clustered to represent its scarcity.
Terrorism:

New terrorism spy missions can lower a city's population by 2 (to a minimum of 1) and increase unhappiness by 2 for 10 turns. Police stations, spies, and garrisoned units help to prevent such missions.
Police Station Changes:

In addition to the -25% war wariness, the police station now reduces the effect of drugs (+1 health for opium, cocaine). It also helps to prevent terrorist attacks.
Hospital Changes:

Hospitals reduce the effect of tobacco by +1 health.
