1. The City Size Box dots are optional. If they are absent, the box is positioned in the top left corner. Since both the orange (with the original palette) and the turquoise (with the units.gif palette) dots are impossible to maintain, I recommend just getting rid of them.
2. Flags don't render if their vertical dot is positioned above the tile.
